Blackheads are one of the most common concerns we see in our treatment room, and one of the most misunderstood. Most people assume they're dirt and try to scrub them away. The truth is, blackheads have nothing to do with how clean you are, and scrubbing harder usually makes them worse.

Here's what's actually going on inside your pores and how we help clients keep them clear.

What is a blackhead?

A blackhead is a type of non-inflamed acne called an open comedone. It forms when oil (sebum) and dead skin cells pack together inside a pore. Because the top of the pore stays open to the air, that plug oxidizes and turns dark at the surface.

So that dark dot isn't dirt. It's oxidation, the same thing that happens when a cut apple turns brown.

Blackheads vs. sebaceous filaments

Those small gray or tan dots across your nose and chin may not be blackheads at all. They're often sebaceous filaments, a normal part of how your pores move oil to the surface. Filaments are softer and lighter in color, and they refill within days after they're extracted because they're doing a job.

Blackheads are more compacted and darker. The good news is that both respond to the same approach: keep oil flowing, keep dead skin from building up and keep your skin barrier healthy.

Why blackheads form

Congestion rarely comes from just one thing. It's usually a mix of:

  • Extra oil. Genetics, hormones, stress and inflammation can all turn up oil production.
  • Slow cell turnover. When dead skin cells don't shed on time, they mix with oil and plug the pore.
  • Hormone shifts. Puberty, your monthly cycle, PCOS and ongoing stress all affect oil and pore behavior. (More on that in How Hormones Affect Your Skin and Acne.)
  • Pore-clogging products. Heavy oils, some makeup and the wrong sunscreen can trap debris.
  • A damaged barrier. Over-washing, harsh scrubs and too much exfoliation tell your skin to make more oil, not less.

Why pore strips and DIY extractions backfire

Pore strips and squeezing can pull out what you see, but they don't fix why it's there. Over time they can:

  • Damage the lining of the pore
  • Trigger inflammation and breakouts
  • Stretch pores so they look bigger
  • Weaken your skin barrier and cause oil to rebound

That's why blackheads often come back faster and more noticeable after aggressive at-home removal. On deeper skin tones, squeezing can also leave dark marks that last far longer than the blackhead would have.

How we treat blackheads long term

Real pore care is about regulating the pore, not attacking it.

  • Cleanse gently, twice a day. You want to remove excess oil without stripping. Face Reality Ultra Gentle Cleanser is a favorite for congested skin that's also easily irritated.
  • Exfoliate chemically, not with grit. Ingredients like salicylic and mandelic acid work inside the pore to dissolve buildup. Face Reality Salicylic Serum is one of our go-tos for blackheads.
  • Add vitamin A over time. Retinoids keep cell turnover on schedule so dead skin doesn't build up in the first place. Learn more in Topical Vitamin A: The Powerhouse Ingredient for Clear Skin.
  • Hydrate, even if you're oily. Dehydrated skin overcompensates with more oil. A light gel like Face Reality Hydrabalance adds water without clogging.
  • Get professional extractions. Regular facials with extractions clear compacted pores safely and protect your barrier while you do it.

Can you get rid of blackheads for good?

Your pores aren't going anywhere, and your skin will always make oil. That's a good thing. The goal is balanced oil flow, clear but working pores and a strong barrier. When those are supported, blackheads and filaments become much less noticeable and far easier to keep under control.

One surprise for many clients: stubborn blackheads are often caused by too much exfoliation, not too little. Hydration, calming ingredients and barrier repair matter just as much as exfoliating.
